When and how should I complete the Gateway forms?

The gateway meeting, associated forms, importance of accuracy, and how to troubleshoot missing forms or edit errors after signing.

As you near the end of your apprenticeship, you'll have a crucial "gateway meeting" with your line manager and your Multiverse coach. This meeting confirms you're ready for your End-Point Assessment (EPA) and marks your last day of formal learning. On this day, you'll receive important gateway forms.

Understanding your Gateway forms:

On the morning of your gateway meeting, two forms will typically be sent automatically via email to you, your line manager, and your coach:

  1. The "gateway form"

  2. The "off the job declaration form"

It's very important that everyone – you, your line manager, and your coach – carefully reviews these forms for accuracy. If any errors are found after a form has been signed, the form will need to be corrected, resent, and re-signed by all parties. If you need help completing the form, your coach will be able to support you during your gateway meeting.


Troubleshooting common issues

I haven't received my gateway forms:

  • If the gateway forms haven't arrived on the morning of your scheduled gateway date:

    • First, please check your email's junk or spam folder.

    • If they're still not there, contact the Multiverse support team, and they will arrange for the forms to be reissued to you.

I've signed a gateway form, but now I see an error/need to edit it:

  • For security and record-keeping, once a form is signed, it cannot be directly edited. However, if you spot an error or need to make an amendment after signing:

    • Contact the Multiverse support team.

    • They will be able to reopen the form, allowing you to make the necessary edits. The form will then need to be re-signed.
