Your End-Point Assessment Organisation (EPAO) will send you the work-based project form, usually in their first email to you, kicking off your work-based project. Think of it as a seal of approval. By signing it, you and your manager are confirming that you did the work yourself and that it meets the requirements.

Who signs the Work-Based Project Declaration Form?

You and your manager both need to sign and date the work-based project form. It's best if itโ€™s your direct manager, as they've likely seen your hard work first hand. But don't worry if they're away โ€“ another manager or supervisor can step in and sign for you.
