Overview

Towards the end of your apprenticeship, you will have an official gateway meeting between you, your line manager and your coach; a declaration that you are ready to begin your End-Point Assessment. This will be the day that you submit your portfolio and it signifies your last day of learning.

This article will help you understand the forms that you will receive on gateway day and how they need to be filled out. You’ll also understand more about what to do if you don’t receive them.


On the day of gateway, you will receive 2 forms (or more) depending on which programme you are enrolled onto and the End-Point Assessment Organisation you are registered with.

The Gateway forms are sent out automatically but occasionally we may need to manually send them out to you.

The same form would be sent out to you, your coach and your line manager.

It is important to note that all three parties need to doubly check the gateway form and ensure everything has been entered correctly. Once the form has been signed, if there is an error (your coach would usually notify us of this), it will need to be re-sent out and signed by all three parties again.

  • When will I receive the gateway forms?

Gateway forms are sent out on the morning of your scheduled gateway date, if you have not received these please check your junk mail.

  • I need to change my own or my line manager’s email address linked on the gateway forms, can this be updated and forms resent?

Please get in touch with the support team and mention the email address you would like the forms to be sent. We can then arrange for the forms to be updated and sent out.

  • I cannot edit the form once I have signed, why is this?

If you need to amend the form once you have already signed it, please reach out to the support team so we can re-open the form so you can edit it.
