The induction form will show as a 'task' on your homepage once you have been accepted onto an apprenticeship programme and are preparing to be onboarded. Please complete this before your Flying Start.


Why am I asked for personal details on the Induction Form?

As you embark on this exciting apprenticeship journey, you might be wondering why we ask for certain details in the induction form. Rest assured, each piece of information plays an important role in ensuring a smooth and compliant apprenticeship experience. Let’s break it down:

1. Your National Insurance Number

Providing your national insurance number is crucial as it is related to funding from the government to support your apprenticeship. By reporting your NI number, we can demonstrate to the government that you are a UK taxpayer, which is part of the eligibility criteria for accessing these funds.

2. Address and Mobile Number

Your address and phone number are required for our Individual Learner Record (ILR) and are essential for maintaining accurate contact information. Your address also helps us verify your Unique Learner Number (ULN) and confirm your residency according to the Education and Skills Funding Agency (ESFA) funding rules.

3. Date of Birth

Your Date of Birth (DOB) is another important piece of information we collect. This is necessary for our Individual Learner Record (ILR) submission, as it plays a significant role in determining access to funding. Additionally, this data helps us identify apprentices under 18 and allows us to facilitate access to the Care Leaver's Bursary if applicable.

4. Next of Kin Details

As a training provider, we have a responsibility to ensure the safety and wellbeing of all our apprentices. Collecting next of kin information is a critical part of our safeguarding protocols. It ensures that we can reach out to someone important to you in emergency scenarios.

Question: Where do I find the Induction Form?

Answer: The Induction Form appears in the Tasks section of your homepage at home.multiverse.io. It shows once you have been accepted onto your programme and are preparing to onboard.

Question: When do I need to complete the Induction Form?

Answer: Complete the Induction Form before your Flying Start. You can complete it alongside your Onboarding Modules.

Question: I can't see the Induction Form on my homepage

Answer: If an email prompted you to complete your Induction Form but you cannot see it as a task, you have most likely already completed it. The form asks for basic details such as your date of birth and National Insurance number. If you do not remember completing it, contact the Support team.

Question: I have completed the form but it still shows as a task. What should I do?

Answer: Check that you selected the final Submit button, not just filled in the fields. If the task remains after you submit, contact the Support team.
